Trenton Health Team and St. Francis Medical Center, Trenton, NJ - (2022) As a key member of the GMPHP, Trenton Health Team and St. Francis Medical Center were interested in taking a deeper dive into the specific needs in the City of Trenton, the largest city in Mercer County, New Jersey. 35th Street worked with the Trenton Health Team and St. Francis to refine reporting to demonstrate the underlying inequities that drive the disparities between Trenton and its neighbors. 35th Street Consulting crafted Trenton-focused CHNA, CHIP and Summary CHNA document report for St. Francis Medical Center and for Trenton Health Team 

 

South Jersey Health Partnership, NJ

South Jersey Health Partnership, Camden, Gloucester and Burlington Counties, NJ (2013, 2016, 2022) Facilitated a collaborative research and visioning process between the three health networks and three county health departments comprising the South Jersey Health Partnership. Created individual CHNAs for each of six healthcare partners including: Cooper University Health , Jefferson Health, Virtua Health, health departments of Camden, Burlington, Gloucester counties.

 

Hackensack Meridian Health, NJ

Hackensack Meridian Health, multiple locations, NJ – (2022) Conducted statewide, virtual visioning sessions with more than 100 community partners engaged and participating to set priorities for action for the 15-hospital network throughout New Jersey. Created a system-wide CHIP and individual CHIP plans for each of the 15 Hackensack Meridian Health hospitals. 

 

Community Health Improvement Partnership (CHIP) of Bergen County, Bergen County, NJ

Community Health Improvement Partnership (CHIP) of Bergen County, Bergen County, NJ – (2022-2023) Conducted focus groups and key informant interviews with stakeholders throughout Bergen County on behalf of all 7 Bergen County hospitals (Hackensack University Medical Center, Pascack Valley Medical Center, Christian Health, Valley Health, Bergen New Bridge, Holy Name, Englewood Health) and Bergen County Department of Public Health and facilitated a virtual prioritization session with more than 70 participants as integral parts of the CHNA process. Conducted a hybrid community engagement session with 120 in-person + online participants to present findings and priorities in collaboration with community partners and other consultants. Completed individual CHIPs for each hospital plus a county-wide CHIP on behalf of the collaborative. 

 

Greater Mercer Public Health Partnership, Mercer County, NJ

Greater Mercer Public Health Partnership, Mercer County, NJ – (2020) GMPHP represents a collaboration of the four hospitals, 12 health departments, and 50+ community partners serving Mercer County New Jersey. 35th Street Consulting led their CHNA in 2020 (at the height of the COVID-19 pandemic), honing our virtual facilitation methods, and implementing data in real time to assist in community vaccination and response efforts. 

 

Health Care Council of the Lehigh Valley, Lehigh and Northampton Counties, PA

Health Care Council of the Lehigh Valley, Lehigh and Northampton Counties, PA (2016) 35th Street Consulting provided project management and research, leading the Health Care Council of the Lehigh Valley (HCCLV) to create a rigorous, readable, and compliant collective CHNA for the entire Lehigh Valley in 2016. The HCCLV comprised all five local hospital networks (Lehigh Valley Health Network, St. Luke’s University Hospital Network, Good Shephard Rehabilitation, Sacred Heart Hospital, KidsPeace), a local FQHC (Neighborhood Health Centers of the Lehigh Valley), and the Allentown and Bethlehem Health Bureaus.

35th Street Consulting, LLC partners with the Dorothy Rider Pool Health Care Trust and the Health Care Council of the Lehigh Valley to create the Health Profile for the 2016 Community Health Needs Assessment for the Lehigh Valley.
